Our commitment
Star Living Care LLC is committed to protecting the privacy and security of protected health information (PHI). Our staff are trained on HIPAA privacy and security requirements at hire and annually thereafter.
How we use health information
We use and disclose PHI to provide and coordinate care, arrange treatment with physicians, pharmacies, home care and hospice partners, obtain payment for services, and carry out required operations such as quality review and state licensing oversight.
Disclosures that require your authorization
Uses and disclosures not described here, including most marketing communications and any sale of health information, are made only with your written authorization. You may revoke an authorization in writing at any time.
Your rights
You have the right to inspect and receive a copy of your record, request corrections, request restrictions on certain uses and disclosures, request confidential communications, receive an accounting of disclosures, and receive a paper copy of our Notice of Privacy Practices.
Safeguards
Paper records are kept in locked storage with limited access. Electronic records are protected with unique user accounts, access controls and encryption. Vendors who handle PHI on our behalf sign business associate agreements.
Breach notification
If a breach of unsecured protected health information occurs, we notify affected individuals and the U.S. Department of Health and Human Services as required by the HIPAA Breach Notification Rule.

Contact our Privacy Officer at (952) 977-9101 or hello@starlivingcare.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights, or with the Minnesota Department of Health. We will never retaliate against you for filing a complaint.
